It is a 272-bed academic health science centre dedicated to delivering exceptional patient care, providing a breadth of services from emergency care to sophisticated brain surgery.

The PATIENT CARE ASSISTANT participates in direct and indirect patient care activities in order to carry out the multidisciplinary care plan.

The scope of activities are directed and assigned by the RN or RPN. To support the provision of excellent patient care, the principal responsibilities of the PATIENT CARE ASSISTANT include : performing basic patient care functions which contribute to the implementation of the multidisciplinary care plan;

assisting with patient nutritional needs; contributing to the patient information collection / documentation process; practicing infection control and safety measures to maintain the highest standard of patient care / safety;

ensuring a clean and safe environment for patient care / treatment; contributing to patient assessment; performing transportation function;

performing departmental duties, following strict adherence to confidentiality policies; performing cross-functional and other duties consistent with the job classification or clinical specialty, as required.

QUALIFICATIONS :

Completion of Grade XII, or equivalent

Completion of a Health Care Aide / PSW Program at the certificate level required; qualified internal applicants with no PSW certification have one (1) year to successfully complete a recognized program

Recent six (6) months direct patient care experience

Completion of a Basic Cardiac Life Support (B.C.L.S.) program

Ability to speak, read, write and comprehend the English language

Knowledge of the Occupational Health & Safety Act and other relevant legislation

Knowledge of the Workplace Hazardous Materials Information System (W.H.M.I.S.)

Knowledge of Hospital and departmental policies, procedures, guidelines, protocols and practices

Knowledge of Emergency Assistance Code

Demonstrated independence, a positive attitude and the willingness to work effectively in a team

Ability to facilitate unit functioning and smooth patient care flow

Ability to maintain a consistent level of productivity and efficiency

Ability to perform responsibilities in a courteous manner and produce high quality work while meeting deadlines in accordance to UHN standards

Awareness of Patient Centered Care

Exercise initiative and good judgment with ability to multi-task

Compliance with confidentiality requirements under UHN Privacy Policy

Client service oriented, with the ability to effectively work with diversity and appreciate that people with different opinions, backgrounds and characteristics bring richness to the challenge or situation at hand

Ability to meet the physical demands of the job required including : ability to stand, stoop, walk, bend, lift, push, pull and reach without any physical discomfort on a continuous basis;

transferring patients with appropriate assistance; ability to stand for extended periods of time; ability to lift in excess of 40 kg

May be required to work at multiple areas as well as rotating shifts in emergency situations

Social Worker

In the provision of patient and non-patient care activities, the primary responsibilities of the SOCIAL WORKER include : assessing patients for the purpose of evaluating the need for service, the nature of the problem, and formulating recommendations or treatment plans;

implementing an intervention plan with patients, family members, significant others, and the multi-disciplinary team for the purpose of facilitating changes in behavior, attitudes, feelings and / or the environment;

facilitating family / team meetings; reassessing and modifying the intervention plan as required; liaising with appropriate community agencies;

completing applications for Alternate Level of Care (ALC) facilities, establishing contact with members of the interprofessional team and community agencies to obtain, provide or exchange information relating to patients' psycho-

social functioning and health care in the past, present and future; rendering services for the immediate benefit of the Hospital and the community;

providing clinical teaching to Social Work Students, Medical Students, Professional Colleagues at University Health Network, Faculties of Social Work and in the community;

participating in relevant research activities, projects, etc.; performing activities required for the effective and efficient operation / maintenance of the Social Work professional group;

performing other duties consistent with the job classification, as required. The has knowledge of the Health Care Consent Act and be familiar with Advance Care Planning.

JOB SUMMARY :

Completion of a Masters Degree in Social Work (M.S.W) or equivalent required

Registration with the Ontario College of Social Workers and Social Service Workers required

Membership in the Ontario Association of Social Workers preferred

Two (2-3) years experience working with an interprofessional team in a health care setting preferred

Excellent verbal and written communications skills

Excellent interpersonal, time management and organizational skills

Excellent problem solving skills

Research experience / education an asset

Student education / supervision an asset

Knowledge of policies, legislation, program's and issues related to social welfare institutions and services

The items shipped in each warehouse can range in size from books and cosmetics to kayaks and flat screen televisions.  As an Amazon associate, there are a variety of jobs you can be trained in and asked to do. Things you should know about working in an Amazon Fulfillment centre:

We are committed to providing one of the safest work environments. This means you will wear a reflective vest, stretch, provide safety tips, and recognize each other for working safely.

We are customer obsessed, and together we focus on the customers to ensure their satisfaction; because meeting customer expectations are a must, quality and productivity are critical. 

Noise levels vary and can sometimes be loud in parts of the building. Hearing protection is available.

A relaxed dress code gives you freedom to move around and feel comfortable (but safe) while working.

Amazon lets customers order whatever they need, whenever they need. Flexibility is key; associates should be open to working voluntary and mandatory extra hours and to working at a fast pace.

Even with climate controls, temperature in some parts of our Fulfillment centers may vary between 16 and 32 degrees, and can occasionally exceed 32 degrees. Especially if you’re working in the truck yard or inside of a trailer on the dock during a hot day.

Some tasks will require standing in one place for extended periods of time (up to 10-12 hours a day), while others involve walking around the facility, including climbing stairs – comfortable, closed-toed shoes are a must!

What does a Warehouse Associate do?

Receives product using frequency scanners, pulls and packs products and unloads shipments from trucks.

Hands-on work. You can expect to handle packages ranging from small envelopes up to boxes weighing up to 49 pounds.

Be on the move. You should be willing and able to operate carts, dollies, hand trucks and other moving equipment in order to move large quantities of merchandise throughout a 10-12 hour day.

Potential requirement to operate powered industrial truck equipment. You may be asked stow and pick items from our racks, which can be approximately 45 feet tall, and sometimes in narrow aisles.

Hourly Pay Rate: $15.75 (Night Shift Premium - Additional $0.50)
